ASSUMPTION AND ACKNOWLEDGMENT OF RISKS AND RELEASE OF LIABILITY AGREEMENT
In applying to enter my son(s), daughter(s) or relative(s) in Aloha Surfing School or skate program I promise to inspect the skate and surfing site, and assure myself that the area is safe for surfing and skateboarding and further agree that I will not participate nor allow the below named minor to participate unless I am satisfied that the area and conditions are safe for surfing or skateboarding purposes. I will also release all rights of my childs camp photos to Tina Hutchings photography and Aloha's Surfing School advertisements .I understand and will comply with all Aloha Surfing School rules and regulations, note existing weather conditions, and do agree that I voluntary assume all risk arising out of surfing or skating activities and conditions relating to use of the surf camp or Abacoa Skate Park. In consideration of your acceptance of this entry, I intending to be legally bound, hereby, for myself, son(s), daughter(s), relative(s), heirs, executors and administrators, hold harmless and release and forever discharge the Aloha Surfing and Skate School, Paul Vargas, Palm Beach County Parks and Recreation, Palm Beach County, Town of Jupiter and Parks, and Aloha Skate & Surfing School's Sponsors and all of their employees, agents, independent contractors, servants and any officials connected therewith, from any and all liability and causes of action for bodily injury, damage or other loss, including but not limited to my own negligence and the negligence of the above named parties, arising out of my or the herein named minor's presence or
participation in the above described event and as described in the Registration form which is incorporated herein in full. I attest that I, my son(s),
daughter(s) or relative(s) is physically fit and is sufficiently trained for surfing/skateboarding and do hereby grant the above named parties such release as described herein. I hereby represent and warrant that I have authority to sign this document on behalf of
